How to create new attestations: Attestation Forms

Learn how to use Corterum to create attestation forms for your staff members to respond to.

Navigate to the attestations wizard

  • First, click Administration>Attestation Forms> ‘plus’ sign.

Describe the attestation set to be created

Next, you will be required to define the set of attestations you wish to create. Specifically, you will be required to:

  1. Give the attestation set a name;

  2. Provide a description for the attestation set (primarily, this is designed so that other users can readily understand its purpose);

  3. Click “next” to proceed to the next screen.

Define the questions to be asked

You should now be on the “Attestations” tab. Here you can:

  1. Add the relevant attestation in the first box (in the form that you would like it to appear in the actual attestation form).

  2. Add any additional information (such as guidance notes or cross-references to regulation) in the second box.

  3. Add more attestations, by simply clicking the “plus” button underneath the attestation and repeating the above process.

  4. Remove attestations, by simply clicking the “cross” next to the relevant attestation.

When you have added the requisite number of attestations, click “next” – this will take you to the “Configure Attestations” tab.

Configure your attestations

Within the “Configure Attestations” tab, for each attestation within a set that has been defined, users have the ability to:

  1. Configure the answer required from the user (either “yes/no”, “red/amber/green” or custom responses configurable by an admin user);

  2. Indicate which response from a user would be regarded as raising a ‘red flag’ and therefore potentially merit further investigation; and

  3. Whether the attestation in question should be regarded as a ‘high priority question’ – with the result that a ‘red flag answer’ would actually require further investigation (e.g. by HR or Compliance).

  4. Choose whether or not to require the user to explain any 'red flag' answers.

  5. To remove an attestation, click the cross

Once you have completed the above process, click “save”.
