How to generate a Form C report, which is a notice of ceasing to perform controlled functions (including senior management functions)

Corterum allows users to generate a Form C report, which is a notice of ceasing to perform controlled functions (including senior management functions)

If an individual ceases to perform their functions within an organisation (for example, due to them leaving the organisation) a Form C report must be produced.

1. To get started, click on REPORTING.

2. Then, select Form C from the left-hand menu.

3. A list of approved persons will then appear.

To produce a Form C report, follow these steps:

1. Select the person for whom you would like to generate the report.

2. Select a date to generate the report from.

3. Select an FCA/PRA contact for this report.

NOTE: Form C asks firms to indicate the number of supporting sheets and support documents as part of the form.

4. Optionally, you can set the number of supporting sheets.

5. If you have any supporting documents, you can set a name for them as well as the mode(s) of delivery.

6. You can also specify any other information, if necessary.

7. To view the document, click GENERATE PREVIEW.

8. The report will then appear.

9. You can download the report as a PDF or DOCX file by clicking DOWNLOAD REPORT.

10. You can also save the report to the document repository by clicking SAVE REPORT TO DOCUMENTS
